Making a Referral

Contact us by phone or email if you would like to discuss a referral before making it, discuss costs, potential courses, the focus of mentoring and what your particular support will look like.

Fill in a referral form including as much detail as possible which can be found here.

Send the completed referral form to info@impactmentoringhq.co.uk

If you have not heard back from us within one week, please give us a call to check progress on your referral as some times in the year are very busy.

Check that the family and young person are aware and supportive of the mentoring before we do our home visit.

How to request a referral form?

Complete and return the referral form sending to info@impactmentoringhq.co.uk

From here Lee our engagement manager will lease with parent or carer and arrange the set-up meeting with the mentee.

The referral is processed and is added to our “waiting room”

Impact Mentoring will acknowledge receipt and respond with any queries - please provide a purchase order number at the top of the form. We cannot commence work until in receipt of this.

Off the back of the meeting Lee will return and liaise with Sue our scheduling coordinator and advise on his recommendations on the appropriate mentor fit to mentee.

Session is scheduled and the mentor will communicate with parent / carer at least 24 hours before the first session. (Once engaged a mentor messages ahead of each session)
